CONSENT STATEMENT
I, ________________________, hereby give my consent to receive training from StuntX Acadmey- Pune.
I understand and acknowledge the following:
- I understand the nature and purpose of the training programs and activities.
- I have had the opportunity to ask questions and received satisfactory responses.
- I acknowledge the inherent risks involved, including injury or accidents.
- I accept full personal responsibility for any injury, loss, or damage; StuntX Academy shall not be held liable.
- The academy strictly discourages stunts on public roads/unauthorized areas; any such actions are my sole responsibility.
- Wearing complete safety gear at all times during training/practice is mandatory.
- I agree to bear 50% of repair/replacement costs for any extreme damage to academy equipment caused due to negligence or misuse.
- I confirm that I am physically and mentally fit to participate.
- I will follow all trainer instructions, safety rules, and academy policies.
- My personal information will remain confidential unless required by law.
- I understand that violations or unsafe behavior may result in immediate termination without refund.
LIABILITY RELEASE
I release and hold harmless StuntX Academy - Pune, its trainers, staff, and affiliates from any and all liabilities, claims, or demands arising from my participation in the training programs, except in cases of gross negligence or willful misconduct.
